PHYSIOLOGICAL BEHAVIOUR: Anabolic Steroid & Testosterone Effects

1. Direct Anabolic Action

All anabolic/androgenic steroids work primarily by binding to the androgen receptor in muscle cells, triggering a cascade that boosts protein synthesis. Introducing exogenous steroids elevates circulating hormone levels, amplifies receptor activation, and accelerates the repair and growth of muscle fibers.

2. Indirect Hormonal Effects

Beyond direct receptor binding, androgens modulate other anabolic pathways. They enhance insulin sensitivity—improving nutrient transport into myocytes—and can influence local release of growth factors. Effective hypertrophy depends on balancing increased protein creation with reduced protein breakdown and optimized delivery of amino acids and glucose.

3. Anti-Glucocorticoid Activity

Cortisol, a catabolic glucocorticoid, instructs muscle to degrade protein (catabolism). High-dose androgens displace cortisol from its receptor, blunt its catabolic signals, and tilt the balance toward net anabolism. Research suggests steroids may also interfere with cortisol’s DNA binding at the glucocorticoid response element, further protecting muscle tissue.

4. Creatine & Testosterone Synergy

Creatine phosphate (CP) regenerates ATP during high-intensity contractions. Steroid-induced strength gains paired with elevated CP stores boost power output, delay fatigue, and create greater mechanical stress—fuel for enhanced muscle growth and faster recovery.

5. IGF-1 & Local Growth

Insulin-like Growth Factor-1 (IGF-1) amplifies anabolic signaling in muscle. Testosterone replacement increases IGF-1 receptor density in skeletal muscle, enabling stronger local responses to growth stimuli—independent of circulating growth hormone.

6. Testosterone Forms & Binding Proteins

Only ~2 % of testosterone circulates free—active and able to bind receptors. ~45 % is bound to SHBG (Sex Hormone–Binding Globulin) and ~53 % to albumin, which temporarily sequester the hormone. Modifying steroid structure alters plasma protein affinity: a higher free fraction increases potency milligram-for-milligram.

Factors affecting SHBG levels include estrogen and thyroid hormones (↑ estrogen → ↑ SHBG; ↓ thyroid → ↓ SHBG). Elevated androgens lower SHBG, further boosting free testosterone. Co-administered steroids can displace testosterone from binding sites, increasing the unbound hormone pool and potentiating each other’s effects.

7. Estrogen & Aromatization

Aromatase converts testosterone into estradiol in adipose, liver, brain, gonads and muscle. Moderate estradiol supports healthy cholesterol, glucose utilization and stimulates growth hormone release. It also upregulates G6PD—an enzyme in the pentose phosphate pathway crucial for nucleotide and lipid synthesis during muscle repair. Excess estrogen, however, causes water retention, fat gain and gynecomastia, so aromatase inhibitors (Arimidex, Aromasin) are often used to keep estrogen in check.

8. DHT Conversion & Tissue Specificity

5-Alpha reductase transforms testosterone into dihydrotestosterone (DHT), a steroid 3–4× more potent at the androgen receptor. DHT formation in prostate, skin, scalp and central nervous system supports localized androgenic effects—enhancing neuromuscular activation but also contributing to acne and male-pattern baldness. Blocking DHT with finasteride can blunt some neuromuscular and strength benefits.

9. Practical Implications & Application

By understanding these physiological processes, you can:

  • Choose compounds based on desired anabolic-to-androgenic ratio.
  • Manage estrogen via aromatase inhibitors to avoid water retention.
  • Optimize recovery by leveraging creatine and timed nutrient intake.
  • Monitor SHBG and free-testosterone to gauge real-time potency.
  • Use DHT considerations when selecting 5-alpha substrates or inhibitors.
